Beneficiary Form

This form allows you, as an Independent Distributor with Neumi, to designate a beneficiary for your account in the event of your death or permanent incapacity.

Instructions for completing the Beneficiary Form:

1. Download and fill out the form.
2. Sign the form in the presence of a notary public.
3. Print or save a copy for your records.
