      Revocation Form

      This form is used to revoke third-party authorizations for financial transactions, purchases, commission use, and account changes with Neumi.

      Instructions for completing the Revocation Form:

      1. Download and fill out the form.
      2. Sign the form.
      3. Print or save a copy for your records.
      4. Return the form by either sending it via email to support@neumi.com or mailing it to the following address:
